Background Check Disclosure
321 Hoops may obtain a background check in connection with your application for, eligibility for, or service in a coaching position with 321 Hoops.
Depending on the position and applicable law, the background check may include information concerning criminal history, sex offender registry records, identity verification, and other information legally permitted and reasonably related to determining suitability for the coaching position.
If 321 Hoops uses a third-party consumer reporting agency to conduct the background check, the resulting report may constitute a "consumer report" under the federal Fair Credit Reporting Act ("FCRA").
The information obtained through the background check will be used for legitimate screening and safety purposes associated with the coach position and will be handled in accordance with applicable law.
Authorization
By selecting I Agree below, I acknowledge that:
- •I have read and understand the Background Check Disclosure above.
- •I voluntarily authorize 321 Hoops and its authorized background-screening provider, if applicable, to obtain a background check about me for purposes related to the coach position.
- •I authorize courts, government agencies, law-enforcement agencies, and other legally permitted sources to provide information requested as part of the authorized background check, subject to applicable law.
- •I understand that providing this authorization does not guarantee selection, employment, appointment, or continued service as a coach.
- •I understand that I may have additional rights under the Fair Credit Reporting Act and applicable state or local laws.
